Evidence of Citizenship

Certified birth certificate with original seal. This certificate will be submitted with the passport application, but will be returned to your with your passp

U.S. Citizens born abroad (Consular Report of Birth, Certification of Birth or Certificate of Citizenship

Naturalized U.S. citizens (Certificate of Naturalization). |

Note: Expired forms of ID are not accepted. Parents ID will be used for children under 16 years of age. |

Photographs
Two identical 2” x 2” passport quality photographs. These should be taken with the person looking straight forward, head and shoulders only on a plain background. Most places that take pictures will know what you need if you tell them they are for a passport. |
Fees
- $97.00 for adults (passport good for 10 years)

- $82.00 for children 15 years or younger.

Optional $60.00 Expedite Fee (receive passport within 3 weeks).

Expedited fee does not include overnight delivery fees.Contact U.S. Postal Service, Federal Express for cost of these fees.
Checks should be made payable to Clearwater County Clerk or cash is acceptable. (These fees took effect August 19, 2002.) |

Processing Time
- Up to 6 weeks for passport delivery from our office to your address
- Up to 3 weeks for passport delivery from our office to your address when expedited
Those needing expedited service pay the additional fee, plus two-way overnight delivery, and receive their passports within 2 weeks. Those needing their passports in less than 14 days (for travel or to obtain foreign visas) need to make an appointment at the Seattle Passport Agency by calling (206) 808-5700. |
Application Procedure
- 15 years and younger must be accompanied by parent or legal guardian.
- 13 years and older must sign their application in the present of an agent.
- 12 years and younger cannot sign, only parents signature is needed.
